One time Installation charge $95.00. Millry.Net configures modem, makes central office changes to allow DSL to work over subscriber's phone line, installs and connects DSL equipment to phone line. Subscriber connects computer to DSL modem and configures computer. Step by step instructions for configuration are provided. Installation charge is NON-REFUNDABLE after modem configuration and central office changes are made. (Additional charges will apply for inside wiring, if needed.) $95.00 INSTALLATION FEE IS WAIVED IF CUSTOMER AGREES TO RETAIN DSL SERVICE FOR TWELVE (12) CONSECUTIVE MONTHS. TWELVE (12) MONTH AGREEMENT MUST BE SIGNED AND RETURNED WITH APPLICATION FORM. Equipment Costs Lease and Purchase Options are Available Lease Option - Lease a standard DSL modem for $4.99 per month, or a wireless modem for $6.99 per month. One time purchase of Inline filters ($3.00 per desk jack, $4.50 per wall jack), a Network card $14.75 (if needed), and a Two-way adapter $3.00 (if needed) are required. Subscriber agrees to return leased modem and surge protector upon termination of service. Subscriber will be billed for leased equipment that is not returned. Purchase Option - One time equipment purchase consist of a standard DSL modem $69.00, or a wireless DSL modem for $115.00, Inline filters - $3.00 per desk jack, $4.50 per wall jack), a Network card $14.75 (if needed), and a Two-way adapter $3.00.
