High-Speed
Internet Service for Unlimited Connections is provided by Millry Communications
d/b/a Millry Net.
Long Distance Service
for Unlimited Connections is provided by Millry Long Distance. Unlimited
nation-wide long distance applies to calls origininating inside the Millry
Communications service area and terminating to locations within the 50
states. By signing below, you designate Millry Long Distance as your long
distance carrier for interstate and intrastate long distance service,
and affirm that you understand that you must maintain service with Millry
Long Distance to be eligible to subscribe to an Unlimited Connections
plan. If you are not currently subscribed to Millry Long Distance, your
signature below authorizes us to change your long distance service from
your current carrier to Millry Long Distance. You are responsible for
contacting your previous carrier to discontinue any plan agreements that
you have with that carrier.
Inside Wire Maintenance
Service covers repairs to properly installed inside wiring and telephone
jacks. (Properly installed according to Millry Communications’ standards.)
It does not cover cords, telephone sets or any other equipment connected
to the telephone jacks. Inside wire maintenance provides repair service
for existing jacks, but does not include the cost of adding a new jack
at a different location within the residence. Adding a new jack at a different
location within the residence is considered service order work, and will
be billed at the rates found in the company’s tariff and price list.
IMPORTANT
INFORMATION: Plans are available for voice services to residential
customers only. Prices do not include regulatory fees, taxes, 911 charges,
modem lease or purchase, dsl filters or ethernet cards. International
, calling card, incoming toll free, third party, collect, toll info charges,operator
assisted calls and data transmission calls (including calls to Internet
access numbers) are not included and will be billed separately at applicable
rates found in the company's tariff and price list. Plan changes are not
allowed within the first 30 days. Initial connection is free. If a plan
is discontinued and added back at a later date, a $32.25 connect fee will
apply. A downgrade fee of $27.00 is applicable when switching to a lower
speed Internet package. A minimum 12-month commitment is required for
subscription to plans containing high-speed Internet services; customary
charges will apply for early termination. Plans offering high-speed Internet
services may not be available in all areas, and advertised speeds are
not guaranteed. All other written terms and conditions applicable to the
provisioning of service by Millry Communications and the individual services
included under these plans apply. Millry Communications reserves the
right to discontinue or change package plans upon 30 days notice, and
to terminate plans on accounts showing excessive usage not consistent
with normal voice services.
